Effects of light therapy on patients with delayed sleep phase syndrome

Abstract
Background: Light therapy (LT) is a potent tool for the treatment of circadian rhythm sleep disorder. We present the method and treatment effect of LT in patients with delayed sleep phase syndrome (DSPS).

Methods: We enrolled 11 patients (M:F=4:7). Eleven of them had DSPS. Two of them who had post-encephalitic epilepsy were dropped out because they showed photosensitive seizures to LT. Thus the remaining 9 patients received LT with a bright light box for consecutive 7-10 days. The patients sit in front of a ¡°light box¡±, which emits 10,000 lux of full spectrum fluorescent light for the periods of 30-45 minutes daily in the morning right after wake-up. Light was restricted by reducing room light for 1hour before light-off at bedtime. The timing of LT was readjusted everyday to the scheduled wake-up time of each day.

Results: By the LT, the average bedtime was advanced from 02:30 a.m. (range; 01:30-5:50 a.m.) to 23:15 p.m. (range; 22:00-23:50 p.m.) and the average wake-up time from 11:30 a.m. (range; 9:20 a.m.-2:30 p.m.) to 7:50 a.m. (range; 7:20-8:30 a.m.). All patients have maintained the readjusted bedtime and wake-up time schedules for 3.8¡¾2.4 months without additional light therapy. No adverse events of LT were observed.

Conclusion: A short-term LT was able to advance a delayed sleep phase and the advancement of sleep phase has been maintained for more than 3 months.
